We're excited to have you become a part of Saratoga Presbyterian Preschool! Continuing families (returning child or younger sibling), alumni families and church families may register in January. New or first time families may register beginning in February and continues on a "first come first serve" basis until an age-level program is filled. To reserve a spot for a child, the school must receive an application form and a non-refundable $50.00 fee (not necessary for names beyond #10 on the wait list). You will be notified in March of space available for your child.

The registration packet is separate from the parent handbook and will be mailed to you in April, with all forms and September tuition due by June 1. The forms required by the school and our licensing agent, the Department of Social Services, are as follows; the starred items will need to be completely filled out and returned to us before your child can attend school. Please make any copies for your own records prior to mailing back the forms.

*1) Personal History - a form filled out by you that provides a background of your child.

*2) Physician's Report - a form to be completed and signed by your child's doctor.

*3) Identification and Emergency Information - a form designating contact persons in case you cannot be reached.

*4) Blue Immunization Form showing your child's immunization history.

*5) Consent for Medical Treatment - a form giving us permission to seek medical care, is needed, for your child.

*6) Field Trip Permission - allowing your child to participate in supervised outings.

*7) Enrollment Forms Receipt - a form signed by you stating that you received and have a copy of the forms required by the Department of Social Services which include:
a) Child Abuse Pamphlet (beige)
b) Personal Rights (beige)
c) Parent's Rights (beige)

*8) Admission Agreement - a form signed by a parent explaining the terms and policies of our preschool and your obligations as a parent in our program.
